Effective July 1, 2013 the Fulton County Court of Common Pleas, Juvenile Division will no longer provide paper Domestic Relations Forms. The expense for the Domestic Relations Forms has become too great.

Please contact legal counsel for assistance, the Court’s staff cannot assist you in selecting or completing the forms.

Forms for House Bill 130 POA/CAA (PDF) (Grandparents Power of Attorney/Caretaker Affidavit) and Application for Custodian (PDF) are available below on the Fulton County Website. *A Birth Certificate for the child is required to be filed at the time of filing the Grandparents Power of Attorney/Caretaker Affidavit and Application for Custodian.
